Join our Community of Practice
Hospice UK brings together organisations to share their work, build skills, knowledge and develop confidence via the National Transition Community of Practice.
This helps us to transform care for young people with complex and life-limiting conditions. It also helps us extend our reach and further influence partnerships across all care providers and key stakeholders. We regularly learn together, and interact both virtually, and face to face via annual meetings.

Hospice UK and Together for Short Lives are working together to provide up-to-date information about Transition via a quarterly newsletter.
This features events, latest articles, best practice, research and other useful resources.
National Transition Professionals Meetings
Also alongside Together for Short Lives, we chair and facilitate quarterly Transition Meetings for Professionals working in the palliative care sector. This is available to those working across the UK.
These meetings are for professionals from a range of backgrounds who care for young people with complex and life-limiting conditions and their families.

When you sign up to join the network, you will be invited to join our Microsoft Teams online discussion group, where you will be able to share information and chat to fellow members of the group. You will be given instructions on joining the group once you've completed the registration form.
